JOB DESCRIPTION

We specialise in supporting adults with a range of Learning disability, mental health and cognitive impairment. Within this, our service users also present with many complex needs such as alcoholism, Aspergers/Autism, cognitive decline, challenging behaviour and/or medical issues. We support them to live in their own properties, in their own communities: environments they know and trust. We provide practical support with personal care, medication, social assistance and emotional support, to name but a few.

You will need to demonstrate positive Values and Behaviours   in   all interactions, raising awareness and improving practice in respect of Equality   and   Diversity   and   promotion   of   Human   Rights   within   the workplace.

What You’ll Do

As Senior Support Worker with Rightnurse, you’ll take on a dynamic role that allows you to make a real difference every day. Key responsibilities may include:

*Delivering specialist directed support to individuals during acute episodes and responding to mental health crises, de-escalating aggressive or high-risk behaviours through planned and structured positive behaviour delivery
*Delivering and maximising therapeutic support with complex service users through collaborative working and community engagement
*Supporting and monitoring individualised care plans to maximise emotional and mental well-being.
*Advocating for patient rights, ensuring dignity and respect in every interaction.
*Collaborating with multidisciplinary teams to deliver comprehensive and compassionate care.
*Assessing and monitoring at-risk patients for signs of suicidal ideation or self-harm, recording and reporting swiftly to ensure safety.
*Documenting and cultivating clear and concise evidence based care records working with professional integrity at all times
*Work in services to actively promote best practice, build strong trusted professional relationships and empower independence and growth to those you support

What We Need From You

*A professional, values based and a proactive approach to support
*At least six months of UK-based paid experience within a complex care and mental health establishment
*At least 6 months proven experience of delivering successful PBS, MAPPA, Behaviours that challenge planning and support pathways.
*The right to work in the UK.
*An excellent standard of English and effective communication skills.
*Flexibility to travel as required across our Countywide, community-based service delivery.

• Adherence to CQC and Care Act 2014 guidelines, as well as following the Code of Conduct associated with each aspect of our regulated activities
• Working in collaboration with other team members, following instructions provided by management, provide feedback and knowledge informed guidance on complex situation and behaviour planning, whilst upholding the dignity, rights, independence and wellbeing of individuals.
• Be flexible within your role. This includes the occasional covering of night and weekend shift covers.
• Have a positive outlook.
• Be able to use your own initiative.
• Be punctual and reliable.
• Undertake further professional training where required, to assist professional progression and keep up to date with current legislation.
• Attend team meetings and have input.
• Attend regular staff supervision and annual staff appraisals.
• Follow company policies and procedures and keep updated with any changes communicated.
• Follow Health and Safety policies and procedures.
• Keep records and paperwork that are accurate and up to date on individual service users.
• Be polite and respectful to all service users no matter what a scenario holds, and ensuring dignity and privacy is respected.
• To follow and accept the guidelines of your employment contract.
• As a staff member you will be accountable for your actions and be answerable to the registered managers and company management.
• To take on board any other tasks that may be delegated by the managers.
• To cover if needed and if possible, any absence of fellow colleagues through staff sickness and staff holidays.
• To attend internal person-centred support plans and reviews meetings with other professional.
• To be a part of or report any safeguarding issues on service users to relevant agencies and managers.
• To respect and uphold confidentiality.
• Maintain effective communication with multi agencies and the rest of your team.
• To report immediately to the appropriate person, any concerns around wellbeing, illness, accidents, incidents or safety and follow guidelines appropriately.
• To be able to work as part of a team or where is it assessed as a safe to do so as a lone worker.
• A full driving licence, with access to your own insured vehicle is preferable
• Use your own vehicle for appointments or community visits in different locations. For which you will be paid 45p per in support mile.
• Uphold a non-judgemental approach to care that promotes equality, diversity and inclusion.
