Continuum Care (Cornwall)

Registered for Domiciliary Care, Continuum Care provide care in peoples own home from traditional care at home packages through to Outreach Services and Supportive Lifestyles (24 hours a day). In our supportive lifestyles packages we support individuals with Autism, mental health and / or both. Our outreach offers an opportunity to support children and young people alongside our elder care care working the shorter hours packages.
