As a Rapid Response Care Worker for Stayathome Cornwall, you’ll play an important role in providing intervention to help with hospital discharges across Cornwall. On a typical day, you will visit immediate hospital discharges at home, completing their care visits (sometimes involving personal care) for a short period until they move on to more permanent care. As part of your role, you will be required to complete assessments, contact relevant professionals, fill out paperwork and relay information to other teams and departments. This role is located in and around the Truro area with shifts on rotation from Monday – Sunday.

Essential Skills:

NVQ Level 3 in Health and Social Care
Full, clean driving license and own transport
Great communication skills
