How do I nominate?

Please read:
- The awards are open to anyone currently working within the external independent adult social care sector in Cornwall.
- All entries are treated confidentially.
- Entries will be judged anonymously by a panel of independent and impartial adjudicators. Winners will be chosen from a shortlist by the Award Sponsor.
- Please make sure all information is accurate at the time of entry.
- Nominations will be shared with the managers of the shortlisted nominees once shortlisting has been completed.
- All nominees will be informed that they have been nominated and will receive a certificate.
- Proud to Care Cornwall will not be held responsible for the publication of any information contained in the entry that was inaccurate.
- Judging will take place around 6 weeks prior to the awards at the Eden project on 19th September.

For entries via the nomination form:
- Choose the category or categories for which you want to nominate your colleague or care worker. If you’d like to enter them for more than one category, please make a separate entry for each.
- Read the category judging criteria carefully. This can be found on the Awards Page.
- Tell us in 300 words or less why they’re great and deserve an award.
- Fill in the form below before Friday 30th June.
For entries via social media (VIDEO ONLY):
- Follow us on your preferred platform Proud to Care Cornwall Facebook or Proud to Care Instagram
- Record yourself telling us your name, who you want to nominate and for which award.
- Send your video via messenger or upload it and tag us.
- We will get in touch to collect the relevant contact details.
