Nominations

Please read:

• The awards are open to anyone currently working within the external independent adult social care sector in Cornwall.
• All entries are treated confidentially.
• Entries will be judged anonymously by a panel of independent and impartial adjudicators. Winners will be chosen from a shortlist by the Award Sponsor.
• Please make sure all information is accurate at the time of entry.
• Nominations will be shared with the managers of the shortlisted nominees once shortlisting has been completed.
• All nominees will be informed that they have been nominated and will receive a certificate.
• Proud to Care Cornwall will not be held responsible for the publication of any information contained in the entry that was inaccurate.
• Judging will take place around 6 weeks prior to the awards at the Eden project on 19th September.

For entries via the nomination form:

1. Choose the category or categories for which you want to nominate your colleague or care worker. If you’d like to enter them for more than one category, please make a separate entry for each.
2. Read the category judging criteria carefully. This can be found on the Awards Page.
3. Tell us in 300 words or less why they’re great and deserve an award.
4. Fill in the form below before Friday 30th June.

For entries via social media (VIDEO ONLY):

1. Follow us on your preferred platform Proud to Care Cornwall Facebook or Proud to Care Instagram
2. Record yourself telling us your name, who you want to nominate and for which award.
3. Send your video via messenger or upload it and tag us.
4. We will get in touch to collect the relevant contact details.
